Notice of Privacy Practices
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Myriad Dental Studio is required by law to maintain the privacy of your protected health information, provide you with notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect.
Uses and disclosures
We may use or disclose protected health information for treatment, payment, and healthcare operations. Treatment includes coordinating or managing your care with other providers. Payment includes activities needed to obtain reimbursement for services. Healthcare operations include quality assessment, training, accreditation, business planning, and other activities needed to operate the practice.
Other permitted uses may include disclosures required by law; public health and safety activities; health oversight; judicial or administrative proceedings; law enforcement; coroners, medical examiners, and funeral directors; organ donation; approved research; workers’ compensation; and efforts to avert a serious threat to health or safety. We may contact you about appointments, treatment alternatives, or health-related benefits and services. Other uses and disclosures require your written authorization, which you may revoke in writing except where we have already acted in reliance on it.
Your rights
Access and copies
You may inspect and obtain a copy of protected health information in your designated record set for as long as we maintain it, subject to limited exceptions under federal law.
Restrictions and confidential communications
You may ask us to restrict certain uses or disclosures and may request that we communicate with you by alternative means or at an alternative location. We will accommodate reasonable requests as required by law. Some restriction requests are not required to be accepted.
Amendments and disclosure accounting
You may request an amendment to information in your designated record set. If a request is denied, you may submit a statement of disagreement. You may also request an accounting of certain disclosures, subject to legal exceptions and limits.
Paper notice and complaints
You may obtain a paper copy of this notice at any time. If you believe your privacy rights have been violated, you may complain to our Privacy Officer or the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.

Website & SMS privacy policy
When you opt in to SMS messaging, we collect the mobile number you provide, message delivery and interaction data, and records of when and how you gave consent. We use this information to send the messages described when you opted in, maintain compliance records, and monitor and improve the service.
Your opt-in applies only to communications from Myriad Dental Studio. Consent is not shared with third-party businesses for their independent marketing. We may share the minimum information needed with service providers that facilitate message delivery, subject to confidentiality and use restrictions. We do not sell your phone number or SMS consent information.
Message frequency varies and message and data rates may apply. You may opt out at any time by replying STOP. Reply HELP for assistance. We take reasonable measures to protect your information, but no transmission or storage system can be guaranteed completely secure.
SMS terms and conditions
By opting in, you agree to receive recurring informational or promotional text messages from Myriad Dental Studio at the number you provide. Consent is specific to Myriad Dental Studio and is not a condition of purchase. Messages will be related to your interaction with our services.
Message frequency varies. Carrier message and data rates may apply. To stop messages, reply STOP; to request help, reply HELP or contact our office. Delivery depends on your carrier and network, and delayed or undelivered messages are outside our control.
We maintain opt-in and opt-out records as needed to demonstrate consent and compliance. We may update these terms and will communicate significant changes through the website or messaging service. Continued participation after an update indicates acceptance of the revised terms.
